Important Reminders
  1. Complete the form for each switched phone number within 60 days of your switch date. To add multiple numbers, click the blue below the Activation/Order Date field.
  2. Enter your new AT&T Account Number, found in your MyATT account or on your Customer Service Summary. For unified accounts, the wireless account number is required.
  3. Upload a wireless bill from your previous carrier that shows: your switched phone number, device balance, number of completed/remaining payments, a billing date within one month of activation, and that the bill is current (not past due). PDFs are preferred.
  4. If your previous provider splits this information across documents, you may upload multiple files, including screenshots or images of your online account and physical bills. [Click here for examples.]
  5. Your new wireless account and switched line must be active and in good standing for 60 days to qualify. Reward cards ship within 8–10 weeks of your switch date.
  6. Check your reward status or update your documents after 7–10 days at att.com/rewardcenter (log in with your phone number and zip code).
Redemption Form
Please provide a full copy of your previous provider’s wireless bill dated as early as one (1) calendar month of your AT&T wireless service activation date (a full PDF is preferred). If switching multiple numbers, please attach supporting bill attachments for each number.
